How We Deliver More Than Homecare

At Kingsway Care, we believe the best home care happens when everyone works together. Caring for people with complex health conditions is rarely the job of one person or one service alone. It takes communication, teamwork and a shared commitment to helping clients live safely and well at home.

Our team work closely with a wide range of healthcare professionals and specialist services to provide responsive, high quality care tailored to each client’s changing needs.

Supporting Clients With Complex Health Conditions at Home

We support adults of all ages living with a wide range of conditions including dementia, Parkinson’s disease, brain injuries, physical disabilities, life-limiting illnesses and end-of-life care needs.

Working Alongside Occupational Therapists

Partnership working with Occupational Therapists plays a huge role in helping clients remain safe and independent at home.

One client living with dementia and Parkinson’s disease experienced significantly reduced mobility. Working closely with the OT team, we helped source and implement specialist equipment to support rehabilitation and improve safety within the home. Through joint assessments, shared communication and follow-up reviews, we ensured the equipment remained suitable and effective.

The outcome: was greater independence for the client and reduced risk of residential care admission.

In another complex case, we worked alongside an Occupational Therapist and a live-in care provider to support a client living with tetraplegia who required a specialist airway clearance technique. The Occupational Therapist developed a safer method using an overhead hoist and sling system to replace an unsafe manual process.

Joint training sessions ensured everyone involved understood the procedure and could carry it out safely and consistently. 

The outcome: significantly improved safety for both the client and care team.

Falls Prevention Training with an Occupational Therapist

Our CareGivers also receive training from a local occupational therapist for falls prevention and how to safely help a client that has had a fall. Learn more

Helping Clients Return Home Safely From Hospital

Hospital discharge can be an overwhelming and vulnerable time for clients and families. Kingsway Care works closely with hospital discharge coordinators, physiotherapists and hospital-based Occupational Therapists to help make transitions home as safe and smooth as possible.

In one case, we supported a client admitted to hospital following a fall linked to self-neglect. Kingsway Care attended a best interests meeting alongside hospital staff and family members to help professionals better understand the client’s needs, routines and behaviours.

The outcome: This collaborative approach helped create a more appropriate and person-centred discharge plan that supported the client’s wellbeing and long-term safety at home.

Supporting Clients Living With Dementia

We regularly work alongside Memory Assessment Services to support clients and families through the dementia assessment process.

Where appropriate, our team attends initial assessments with clients to help reduce anxiety and ensure accurate information is shared with healthcare professionals. This can support earlier diagnosis, improved care planning and better long-term outcomes.

Coordinating Care With District Nurses

Many clients receiving home care also require ongoing clinical support. We work closely with District Nurses to provide coordinated care for clients with complex medical needs such as:

  • Pressure area care
  • Catheter management
  • Infection monitoring
  • Wound care
  • Medication support

For example, one client with a long-term catheter and recurrent urinary tract infections benefits from regular District Nurse involvement alongside daily support from our care team. 

The outcome: Close communication between professionals allows concerns to be identified quickly and reduces the risk of complications.

Safe Nutrition and Swallowing Support

Some clients require specialist support with eating, drinking and nutrition due to swallowing difficulties or other medical conditions.

Kingsway Care works alongside Speech and Language Therapists and nutrition support services to ensure safe nutrition and hydration plans are followed correctly.

The outcome: This collaborative approach helps reduce risks associated with choking, dehydration and malnutrition while supporting clients to maintain dignity and comfort.

We can help you with specialists diet to prevent choking, dehydration and malnutrition.

Compassionate End-of-Life Care at Home

We understand how important it is for many people to remain at home during end-of-life care.

Our team worked closely with The Martlets hospice-at-home team to support one client through six months of complex end-of-life care. Due to rapidly changing pain management needs, ongoing communication between all professionals was essential.

The outcome: Through coordinated medication reviews and clear information sharing, we were able to provide compassionate, responsive care that enabled the client to remain comfortable at home surrounded by familiar people and surroundings.
